Start the form online
Go to the Special Olympics Michigan athlete registration website using the button above.

Use Jo Brighton and Area 23
Enter Jo Brighton as the Local and 23 as the Area.

Print the health-form pages
The online process will provide pages to print and take to the athlete's doctor.

Return the completed form
After the doctor completes and signs the form, email it to jobrightonsports@gmail.com and turn in a copy to the school or coach.

Welcome to Jo Brighton Special Athletes
For the registration/health form, you will start it online. When filling out the forms, Jo Brighton is the Local and the Area is 23. It will then have you print the pages to take to the doctor to fill out. After it is filled out by the doctor and signed, please email the completed form to jobrightonsports@gmail.com, and turn it in to the school or coach.
